Using this type of form will enable you to capture all the relevant information required by accounts and estimating so that purchase orders and payments can be made to the contractor.

The checklist is also a very good way to cross check the contractor's ABN details, licences (where applicable) and insurances.

Check to make sure all the entities line up. If there are different entities licensed and insured then you may be leaving yourself and your business in a situation where the insurance doesn't cover the entity you have contracted with or the entity you have contracted with is not licensed.

In my many years of working in this industry, I have seen discrepancies across this information hundreds of times and the contractors themselves are usually unaware of the problems created.

Make sure you have all the correct information and all entities line up before contracting with the contractor.
